   •  Attested copy of Form 'B'
   •  School leaving certificate bearing the date of birth of the candidate (Original) duel stamped signed by head of the institution.
   •  Affidavit (For late candidates desirous to take admission in Fuss Part-I)
   To show that the candidates has either not appeared in or is ignoring the result of Fuss( Part-I)
   •  Cancellation certificate of the Board (For candidates ignoring the previous result of Fuss (Part-I)
   •  Three passport size photos.

   Uswa College Islamabad is a project of Jabir Bin Hayan (father of chemistry) Trust. This Trust is a vibrant and effective NGO working in the areas of education, health and poverty alleviation in rural areas of Pakistan. Its major focus area is the field of education. Uswa College Islamabad was established in March 2003 and the first entry of students joined the College on 13 April 2003. The College is a residential institution affiliated with the Federal Board of Intermediate Secondary Education Islamabad. Regular admission in the College is made in class VII. Limited admissions may be made in class XI (pre-Medical, pre-Engineering and from 2010 Computer Sciences, only) subject to availability of seats.
