   It could be that the patch reconstructed the internal inguinal ring just a little too tightly and/or the hydrocele may have been overlooked and is now causing a problem.  
   
   I would venture it is time for a second opinion from another surgeon. Obtain a copy of all your records from the operating surgeon and the urologist, including the reports about the hydrocele, prior to the appointment.  That way the consulting surgeon will have all the information.  I favor second opinions from surgeons outside the community, i.e. from a large teaching center. They tend to be more dispassionate.
