   In order to activate your optimum memebership reward card, you will have to access the optimum reward online activation page, there you will have to enter the account numberf printed on your optimum reward card, then you will have to enter your name. Finally you will have to enter your billing address ZIP code in the box provided. Click "Submit" to complete your Optimum Rewards card activation.
