Formalities at Air India for an over-sized who requires an additional seat

My cousin is too fat and he cannot sit on the normal Air India plane seats. My friend told me to book an oversized seat for him. Please tell me how to get an oversized seat at Air India?

    A request for an additional seat will have to be done at the time of reservation From Air India, wherein the necessary remarks will be made in your reservation.  You will be issued only one Air India ticket, with payment for the two seats shown accordingly.
