How to change applicant name of NADRA CNIC?

I want to change my NADRA ID card name because I want to join army and they are saying that my name on Matric result card and ID card are not same. Raja Misbah Islam is written on my Nadra card and only Misbah Islam is written on my Matric result card.

    Applicant can apply to change his name by providing NAME CHANGE DEED attested from Consulate / Embassy of Pakistan along with cutting of Add in newspaper to change his name.
    * Partial change in name like Khan, Malik, Chaudhry etc needs no documents as evidence.
    - Name of any person who has been died / deceased can not be changed without Court Orders.
