The answer to this question depends on a couple of factors, mainly what your threshold for pain is along with the method of piercing. If you have a high threshold for pain (broken bones, bad injuries in your past etc.) then a cartilage piercing should not be overly painful.

From what I have heard, the actual piercing process can range in pain from no feeling at all to slight discomfort during and shortly after. Others, who have had piercings from a gun, say that it hurts worse than soft tissue piercings, like ear lobes.

In the end, it is important to get pierced by a professional, with a needle being a preferred method over a gun, which can cause pain for cartilage piercings.
